Site Visit Summary

My site visits with Dr. St Martin were straightforward and great learning when it comes to psychiatry. During my first evaluation, I did a patient that I suspected was going through an adjustment disorder but during my presentation he pointed out some important things I must consider as a PA. Dr. St Martin mentioned to me that this patient had an intellectual disability and due to this, it would be difficult to assess if it really was the diagnosis that I thought was correct.  He also mentioned that I need to have a more thorough look into the patient’s psychiatric history (prior admissions, relationship with family, risky behaviors) and is something I took upon myself to work on during my last two presentations. These included a women that was experiencing a manic episode due to her bipolar type 1 and a patient that was experiencing signs of borderline personality disorder. I presented my third H&P and Dr. St Martin had notice my improvements that was mentioned during my first evaluation. He did mention that I missed an important diagnosis that I should have included (antisocial personality disorder) and shows that I still have much to learn from psychiatry. He ultimately told me that he would have thought of my diagnosis of BPD and the treatment seemed appropriate for the patient. This makes me happy as I know I’m improving and all this experience is shaping me to be a great future PA. Ultimately, I think Dr. St Martin was very pleased with my H&Ps, drug cards and my journal entry (DBT for BPD) for this rotation.
