I decided to pick this article for my second H&P that involved a bacterial sinusitis diagnosis for an 81-year-old female. This article was a double-blind, comparative-effectiveness randomized clinical trial conducted at Albany Medical Center, Cohoes, New York in 2021. Its purpose was to determine if high-dose amoxicillin plus clavulanate is superior to standard-dose amoxicillin plus clavulanate in treating acute bacterial sinusitis in adults. This directly related to my H&P as my patient received Augmentin, but this research aims to see if a higher dose of the same drug would be more effective in recovery. It was found that there was no significant difference in the primary efficacy outcome between the standard-dose and high-dose groups at day 3 and suggested that adults with acute bacterial sinusitis did not benefit from taking high-dose compared to standard-dose amoxicillin plus clavulanate. Also, as I mentioned during my finale evaluation, I found some interesting secondary findings from this article. The author concluded that the frequency of diarrhea and vaginal yeast infections were quite high in those with a higher dose of the medication, patients who reported they were prone to adverse effects were more likely to experience them and finally that there was a large number of patients with presumed penicillin allergy that shows the need for more widespread testing for penicillin allergy as many of these patients don’t necessarily have a true allergy to penicillin. This was emphasized in one of my didactic classes last fall on the importance of a true allergy, as second and third line options often have worse side effects.
